Delete account request

If you wish to delete your account, use the following FORM

Note:

1. The details should be the same as on the POI and POA documents that you have provided for account verification.
2. The KYC data and other data with AMCs and RTAs will not be deleted for a certain duration due to regulatory and taxation requirements.

Form contents –

1. Your First and Last name
2. Your Date of Birth
3. Your full mailing address on file with your Capitalyze® account
4. Your Capitalyze® ID
5. Registered phone number
