If you are unable to update your transaction account registered with Capitalyze®, use the following FORM

Note: The details should be the same as on the POI and POA documents that you have provided for account verification.

Form contents –

1. Your First and Last name
2. Your Date of Birth
3. Your full mailing address on file with your Capitalyze® account
4. Your Capitalyze® ID
5. Current Account linked – Bank, A/c number, IFSC code
6. New Account to be linked – Bank, A/c number, IFSC code
7. Registered phone number
