Add a beneficiary to your account

It will take around 7 working days to add a beneficiary to your account. You will be notified on your app and on the registered email.

In order to do so:

• Select the FORM
• Fill in the requested information (The form is editable, so you can type everything before you print it.)
• Print out the form
• Print and sign your name at the bottom of page two
• Double-check the form to ensure all information is accurate and complete

Note: The details should be the same as on the POI and POA documents that you have provided for account verification.

1. Your First and Last name
2. Your Date of Birth
3. Your full mailing address on file with your Capitalyze® account
4. Your Capitalyze® ID
5. Beneficiary First and Last Name
6. Beneficiary Date of Birth
7. Beneficiary Relation
